4K laparoscopic hysterectomy and adnexectomy

Indications

Hysterectomy and adnexectomy are suitable for those over 40 years old with serious conditions and no reproductive requirements.

 

Contraindications

1. Hysterectomy alone is not suitable for patients with uterine fibroids combined with malignant tumors in the adnexa, endometrial cancer stage II or above or cervical cancer stage Ib or above.

2. Acute pelvic inflammation.

 

Preoperative preparation

1. Open the broad ligament first, and remove the varicose venous plexus.

2. Perform hysterectomy routinely. If the patient is younger, subtotal hysterectomy should be considered to preserve one or both ovaries. If the menopause is approaching and the ovarian veins have severe varicose veins, they should be removed together with the appendages.

Anesthesia and positioning

Continuous epidural anesthesia is appropriate, and the patient is placed in the supine position.

 

Points to note during surgery

1. When cutting the posterior leaf of the broad ligament, care must be taken to avoid injury to blood vessels and cause bleeding. Do not injure the ureter.

2. The veins in the pelvic cavity are thin and easy to break. The operation must be gentle and careful. If there is bleeding, immediately ligate, cut off, and re-separate the next segment.

 

Postoperative treatment

After hysterectomy and adnexectomy, avoid excessive physical activity and abstain from sexual intercourse.
